Headaches and migraines are common health issues that affect millions of people worldwide. Whether it’s a dull, constant headache or a debilitating migraine, the pain can interfere with daily life, making even simple tasks feel overwhelming. While many people rely on over-the-counter medication for relief, Traditional Chinese Medicine (TCM) offers a holistic, natural approach to treating headaches and migraines, addressing the root cause of the problem and promoting long-term health.

My Clinical Experience

This is a very common condition that I see in the clinic. While there are certainly internal factors that can affect headaches like women’s cycles, and blood pressure, the common issue that unites all of them is tight soft tissue in the back/sides of the neck. Posture, stress, and injury all coalesces in the structures of the neck. This causes a reduction in the blood flow to the head and brain, and our energy demanding brains are hungry for a constant supply.

When it deems it’s insufficient the local tissue around the cranium will swell to try and encourage more blood flow to saturate the head, however, if there is a choke point in the neck/upper back the patient is left various types of pain that can last for hours or days. Acupuncture is by far the most effective technique to use to relax the muscles, reduce swelling, and allow the space between muscles to open and bring more blood supply to the head. The internal factors then can be addressed with herbs and sometimes acupuncture to alleviate the underlying problem leading to recurring or cyclical headaches.

Understanding Headaches & Migraines in TCM

In TCM, headaches and migraines are seen as a disruption of the flow of Qi (metabolic energy) and Blood in the body. This imbalance can be caused by various factors, such as:

  • External Pathogens: Wind, Cold, or Dampness can invade the body, block the flow of Qi and Blood, and lead to headaches.
  • Internal Imbalances: Stress, emotional strain, poor diet, and insufficient sleep can cause stagnation of Qi or Blood, or lead to an excess of Heat, both of which contribute to headaches and migraines.
  • Qi and Blood Deficiency: This can result in a lack of nourishment to the head, leading to headaches, especially in those who are fatigued or have chronic health conditions.

Types of Headaches in TCM

TCM categorizes headaches based on their location, nature, and underlying causes. Understanding the type of headache can help in selecting the most appropriate treatment.

  1. Wind-Cold Headaches: Often occur in the front of the head and are accompanied by symptoms such as chills, a stiff neck, and a runny nose. These headaches are caused by prolonged cold and wind exposure on the head, neck, and upper back.
  2. Wind-Heat Headaches: These headaches are usually felt on the temples or the sides of the head and are often associated with fever, red face, and irritability. Wind-Heat headaches are often triggered by external pathogens like allergies or sinus infections.
  3. Qi and Blood Stagnation: These headaches are typically accompanied by a feeling of pressure or a tight, heavy sensation. They can occur on the sides or back of the head and are usually linked to emotional stress, tension, or poor circulation.
  4. Liver Yang Rising: Often causing intense headaches at the top or sides of the head, this type is due to liver Qi stagnation transforming into Heat, leading to upward-rising Yang. It is frequently triggered by stress or frustration. Traditionally this was caused by hypertension, but clinically I see this the least as most people are put on medication for hypertension long before I start seeing them.
  5. Deficiency Headaches: Typically mild but persistent, these headaches are caused by Qi or Blood deficiency, especially in cases of chronic fatigue or anemia. They are often felt as a dull ache in the head. Very common in women with cycle disruption or people who went through a prolonged illness like long Covid. The body is exhausted and the blood pressure is usually too insufficient to keep up with cerebral demand.

TCM Treatments for Headaches & Migraines

The goal of TCM treatment is to restore balance in the body, promote the smooth flow of Qi and Blood, and address any underlying imbalances. TCM uses a variety of treatments, including acupuncture, herbal medicine, dietary recommendations, and lifestyle adjustments.

1. Acupuncture

Acupuncture is one of the most popular TCM treatments for headaches and migraines. By inserting thin needles at specific points along the body’s meridians, acupuncture helps to regulate the flow of Qi and Blood, release tension, and relieve pain. Distal acupuncture can be used for sensitive patients; this will gently stimulate the nervous system to move blood to the head and remove obstruction. For everyone else I use direct trigger point needling to release the muscles from their rigid positioning causing the obstruction. 

2. Herbal Medicine

Herbal remedies are tailored to the specific type of headache or migraine and the underlying imbalance in the body. Some commonly used herbs include:

  • Chuan Xiong (Chuanxiong Rhizoma): Often used to move Qi and Blood, Chuan Xiong is particularly effective for headaches caused by Qi and Blood stagnation.
  • Bai Zhi (Angelica Dahurica): This herb is used to clear Wind and Cold, making it ideal for headaches caused by these external pathogens.
  • Fang Feng (Saposhnikovia Root): A herb used to expel Wind, often used for headaches due to Wind-Cold invasion.
  • Tian Ma (Gastrodia Rhizoma): This herb is beneficial for headaches caused by Liver Yang rising or internal Wind.

3. Dietary Adjustments

TCM emphasizes the role of food in maintaining balance and health. For individuals suffering from headaches, dietary changes can help address imbalances. Some TCM dietary recommendations for headache relief include:

  • Avoid cold and raw foods: Cold and raw foods can disrupt digestion and contribute to internal dampness, exacerbating headaches.
  • Eat warming foods: Spices like ginger and cinnamon can help improve circulation and dispel Cold, preventing headaches caused by these factors.
  • Stay hydrated: Dehydration can trigger headaches, so it’s important to drink enough water throughout the day.

4. Lifestyle Modifications

TCM also advocates for changes in lifestyle to help prevent and manage headaches:

  • Stress management: Practicing meditation, Tai Chi, or Qi Gong can help to reduce stress and promote the smooth flow of Qi, which can prevent tension headaches and migraines.
  • Regular sleep: Ensuring adequate and consistent sleep is crucial for preventing headaches related to Qi and Blood deficiency.
  • Exercise: Gentle, regular exercise promotes the smooth circulation of Qi and Blood, helping to prevent headaches caused by stagnation. This includes stretches that focus on the upper back and neck.
